If there are no other drugs on the formulary that can be used for treatment, your doctor can request coverage for plan approval drugs, or drugs that are not covered, by providing clinical information stating why other formulary alternatives cannot be used.
If your benefit plan includes prescription drug coverage through Care Choices HMO, your ID card will include a copayment amount that you must pay out of pocket for your prescription drugs.

The project was founded as an innovative partnership between employers and state and local government and it uses a mix of private and public funds to pay for new local transit service.
