More than half a million Medicare beneficiaries are in Medicare+Choice plans leaving the market this year, forcing them to find alternative coverage.
The cumulative effect of withdrawals since 1999 has sharply reduced the number of choices available to beneficiaries.
While Medicare+Choice is still attractive to many beneficiaries, total enrollment has been declining since 2000, with an increasing share of beneficiaries returning to traditional fee-for-service Medicare.
The Commonwealth Fund supported the analysis of Medicare+Choice benefits, the Kaiser Family Foundation supported firm-specific analysis, and the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services funded the analysis of disenrollment rates.

Of the 2.2 million persons in the labor force, 68 percent have either no education or have never completed primary education, 19 percent have completed primary education, 7 percent have completed lower secondary, 3 percent have completed upper secondary, and 3 percent have some kind of post- secondary education.
The increase in number of government employees is a policy decision.
This, in turn, is a function of growth in those sectors of the economy along with other factors that will affect manpower needs in the firms.

Oakland, CA---The value of Medicare HMOs to consumers has declined sharply in almost all parts of California, according to the third annual Guide to California Medicare HMOs, a free report for consumers released today by the California HealthCare Foundation and Consumers Union.
In just two years the number of top ranked plans has dropped 86 percent in California.
Consumers need to decide what's most important to them, and use the Guide to California Medicare HMOs to help them find the best match," said Trudy Lieberman, director of Consumers Union's Center for Consumer Health Choices.
Medicare HMOs generally offer more benefits than traditional Medicare, but they may limit their members' choice of providers and require referrals to get care from specialists.

One of our jobs as parents is to help kids make healthy choices so that they stay well now---and grow into healthy adults.
That doesn't have to mean crackers that taste like flavored cardboard or carrot sticks for dessert.
Research has shown that if the adult wears safety gear, the child is more likely to wear it.

If hospital expenditures are to be significantly curtailed under any program, Relman argues, physicians will have to be given incentives for frugality.
Yet extending the prospective payment system to doctors, as is now being considered, may have serious implications for the quality of patient care, he warns.
In a very real sense, hospitals were the workshops for physicians---a convenient place to observe patients, conduct diagnostic studies, and provide treatment with little or no cost to patients as long as they were insured, and with little or no overhead expense to the physician.
Many insurance plans, including Medicare, reimbursed for certain services provided in the hospital but not in the doctor's office.

This program gives states considerable flexibility in designing expanded health insurance coverage for children.
The purpose of this paper is to lay out these factors -- first, those that affect participation; then, those that affect cost per participating child.
Actuarial analysis illustrates both the way various factors will affect costs per covered child in a Medicaid approach as compared to a new program, and how such costs should be estimated.
Under a separate state program, the federal rules specify that the benefits must be at least as generous as one of three "benchmark" private insurance packages described in the law: the standard Blue Cross/Blue Shield package provided to federal employees, a plan offered to state employees, or the most common benefits package offered by an HMO in the state.
