I may be at risk of acquiring hepatitis B virus (HBV) infection.
I have been given the opportunity to be vaccinated with hepatitis B vaccine, at no charge to myself.
However, I decline hepatitis B vaccination at this time.
I understand that by declining this vaccine, I continue to be at risk of acquiring hepatitis B, a serious disease.
If in the future I continue to have occupational exposure to blood or other potentially infectious materials and I want to be vaccinated with hepatitis B vaccine, I can receive the vaccination series at no charge to me.
Please attach a copy of any vaccination verification if possible.

While our knowledge of effective interventions aimed at preventing transmission of hepatitis C and minimising the impact of HCV infection is improving, much more is still needed.
Transmission in relation to tattooing, skin-piercing, medical and dental procedures, and ual and vertical transmission are all important areas of investigation, but as current information suggests they are responsible for very small proportions of the total burden of infections they are not recommended to be priorities for research.
With the rare exception of extrahepatic disease (e.g. non-Hodgkin's lymphoma, essential cryoglobulinemia), the only causes of death from chronic HCV infection are liver failure, haemorrhage from complications of portal hypertension and hepatocellular carcinoma (HCC).

Hepatitis B is a serious disease caused by a virus that attacks the liver.
Each year in the United States, more than 200,000 people of all ages get hepatitis B and close to 5,000 die of sickness caused by HBV.
Sometimes, people who are infected with HBV never recover fully from the infection; they carry the virus and can infect others for the rest of their lives.

HAV is the most common cause of acute infection, followed by HBV and HCV; however, HCV is the most likely to result in chronic infection.
As with many viral infections, chronic infection with HCV is difficult to eradicate and, until recently, treatments for chronic HCV infection had very low response rates---more than half of all patients failed to respond to treatment.
IFN monotherapy for HCV infection was first introduced over 10 years ago, and the currently available IFNs have similar efficacy and safety profiles.
While pegylated IFN monotherapy is significantly more effective than standard IFN monotherapy, it is not as effective as combination therapy with IFN plus RBV.
Results from these combination trials have been positive, and were described by Dr. Rossi as "the next big step" for the treatment of HCV.
