Hepatitis C: The Next Century UT Southwestern Medical Center
at Austin!
RNA assays or branched DNA assay (Chiron) Quantitative and qualitative,
latter more sens.
Identify that virus is present (use qual assay) Does not reflect severity
of disease!!
Genotypes 5, 6: sporadic, infrequent Cost: $200, but only needs to be
done once.
· Use of other drugs, acetaminophen, etc.
· Not everyone needs or can be treated!
· What is the effect on fibrosis?
· What is available for those who fail therapy?
· What has happened now that we have a working therapy?

We have examined the progression of hepatitis C virus (HCV)
infections by gene expression analysis of liver biopsies in acutely infected
chimpanzees that developed persistent infection, transient viral clearance,
or sustained clearance.
Transient and sustained viral clearance were uniquely associated with
induction of IFN- induced genes and other genes involved in antigen processing
and presentation and the adaptive immune response.
Here, we show that HCV-infected cells successfully induce the transcription
of many antiviral IFN- -stimulated genes, but that this response has little
or no effect on viral titer or outcome, possibly acting via HCV-induced
mechanisms downstream of IFN- -stimulated transcription (16).

In recent years, the treatment of choice is alpha-interferon
in combination with ribavirin.
For patients with failure of this treatment the therapy with consensus
or pegylated interferons, in monotherapy or in combination with ribavirin,
present other possibilities.
responders have not cleared HCV RNA and normalized ALT activity during
alpha-IFN therapy.

hepatitis C virus (HCV) infection have been derived from transfusion
re c i p ients and blood donors.
These studies have shown that the incidence of HCV infection can vary
significantly according to the geographical location.
In South America, Eastern Europe, Mediterranean countries, and Asia the
prevalence has been reported at 1.1--5% w h e reas the highest prevalence
of 20% is found in the Middle East.1 According to data from the Center
for Disease Control and Prevention (CDC, Atlanta, GA), the incidence of
acute HCV in the U.S. remained relatively stable through much of the 1980s
at 18 cases per 100,000 persons per year.

HepC VSG Meetings -- Last Wednesday of each month, 10:30-12:30
PM, BC CDC Building at 655 West 12th (12th and Ash, next to the Cambie
Street City Square Mall- park here) Tom Cox Boardroom on the second floor.
Downtown Eastside HepC Support Group Meetings: Each MONDAY at the Carnegie
Center, 401 Main St., Vancouver from 6-8 PM..
You don't want to believe this could happen to you.
But denying it keeps you from dealing with the disease and doing things
that may improve your quality of life.
Some doctors think that if your liver enzymes (ALT and AST) are normal
then you should be fine.

series, free of cost to you.
materials, which includes the risk of acquiring Hepatitis B virus (HIV)
infection, I have been informed about and offered the opportunity to receive
the Hepatitis B vaccine (to be paid for by my current employer).
If in the future, while actively working with NovaPro, Inc., if I continue
to have occupational exposure to blood or other potentially infectious
materials and I want to be vaccinated with Hepatitis B vaccine, I can
receive it at no charge to me.

Hepatitis C virus (HCV) infection is found throughout the world
with prevalences in blood donors ranging from 0.4% (US/U.K.) to more than
14% (Egypt).
Therefore, the RNA dependent RNA polymerase of HCV has been selected as
a key target for developing specific antiviral therapy.
Structural studies Wild-type and mutant HCV polymerase (J4 derived NS5b;
Mr 66,000) is bacterially expressed, yielding about 20mg of pure HC-J4
NS5B per litre LB medium.
Structure-based drug design and high-throughput screening Using the programs
SPOCK (J Christopher, Texas A&M) and SPROUT (AP Johnson, Leeds), several
small cavities in the vicinity of the active site have been characterized
with regard to possible binding sites for highly specific antivirals.
MHV

Sheets/MHV.pdf
Background: Mouse hepatitis virus is probably the most important
pathogen of laboratory mice.
Although the infection generally causes no overt clinical signs, it can
cause profound changes in the immune system, affecting the interpretation
of a wide variety of experimental results.
Approximately 25 strains or isolates of MHV have been described.
Clinical Signs: Clinical infection occurs when the virus is introduced
into a naïve colony.
The diagnosis can be strengthened by demonstration of typical lesions
in clinically ill animals.
· Decrease in the number of hepatic sinusoidal endothelial cell fenestrae.
Prevention: To prevent this disease, obtain replacement stocks from sources
that are known to be free of disease.

Take an active role in your health care.
How can you prevent the spread of Hepatitis C to others?
The hepatitis C virus is usually spread by blood-to-blood contact with
infected blood.
The risk of passing on the hepatitis C virus via other body fluids such
as breast milk, saliva, semen or vaginal secretions is low but is not
zero.
At this time, there is no vaccine to prevent hepatitis C infection.
Clean twice with bleach then rinse twice with water.
· Other people may have a brief illness with symptoms of hepatitis
usually appearing six to nine weeks after they have been infected with
the virus.

All diagnoses of acute non-A, non-B hepatitis (a catch-all
category that by definition includes hepatitis C) are reportable to the
LHD by telephone within one working day of diagnosis or suspected diagnosis.
Absent evidence of acute clinical illness, however, evidence of exposure
to the hepatitis C virus per se (e.g., detectable anti-HCV antibodies)
is not reportable to the OHD.
1. Report all confirmed and presumptive (but not suspect; see definitions,
§3) cases to the OHD by the end of the calendar week of initial report.
HCV is one of several, often poorly characterized causes of hepatitis
other than HAV and HBV (i.e., non-A, non-B [NANB] hepatitis).
Antibodies develop after infection, but are not protective.

Fox Army Health Center has received information that certain
lots of Hepatitis A vaccine were identified as being ineffective and need
to be repeated.
If you have received Lot # 0507L or Lot # 0716L of the Hepatitis A vaccine,
which was dispensed from 14 October - 18 December 2001, please return
to our Immunization Clinic for a repeat vaccination.
If you are unable to determine if you have received this vaccine, or if
you have any questions, please check with your Primary Care Manager at
your next regularly scheduled appointment.
HEPA
It is transmitted in food or drinks that have been contaminated
with feces from a person infected with hepatitis A, such as when food
is prepared by an ill person whose hands are contaminated.
The virus may be in untreated stream water or contaminated public water.
Hepatitis A is NOT spread by coughing, sneezing, or by sharing a glass,
eating utensil, or can of soda.
Insist that everyone wash their hands after going to the bathroom, changing
diapers, or before preparing food.
If someone in your household has hepatitis A, get an injection of IMMUNE
GLOBULIN as soon as possible (within two weeks of exposure) from your
health care provider or health department.
