ob_handout6.04rritable bowel syndrome (IBS) is a common problem, and as many as two out of every 10 people in the United States suffer from this disor-der. You may also have heard IBS referred to as spastic colon, mucous colitis, spastic colitis, nervous stomach, or irritable colon. To understand IBS, first you should know that the word "bowel" in IBS refers to the lower (large) intestine--the part of your body where stool is formed, water is absorbed, and mucus and fluid are secreted before the stool is excret-ed. The term "syndrome" refers to a collection of symptoms associated with the condition. - Unusual difficulty passing stools, such as needing to strain, having an urgent need to move your bowels, or feeling that stool remains in your bowel after a bowel movement Irritable bowel syndrome itself is not known to be related to colon cancer. 6/04 Irritable Bowel Syndrome It influences how often and how intensely the bowel con-tracts and relaxes, and it stimulates the nerve endingsIBSWhat is irritable bowel syndrome? Symptoms of Irritable Bowel Syndrome (IBS) include pain in the lower abdomen (below the stomach area), changes in bowel habits with frequent, urgent diarrhea or constipation, bloating, and cramping. It is a medical disorder of the lower `gut' (the stomach and intestines), which is one part of the gastrointestinal (GI) tract. For some people it begins in childhood with a `sensitive stomach' that develops into more intense symptoms in adulthood; while for others, the GI problems start suddenly during a period of stress or persist after a parasitic infection in the stomach or bowel. Irritable Bowel Syndrome is very common. Over one-third of the patients that are seen by gastroenterologists (the `gut' specialists) have Irritable Bowel Syndrome. Once IBS develops, however, the bowel is over-reactive to or easily triggered by a variety of things including diet, stress, emotional state, and even hormone fluctuations. In fact, two-thirds IBS_CoI_ISPOR_USABackground: With a prevalence of up to 20%, Irritable Bowel Syndrome (IBS) could cost the German Sickness Funds (GKV) more than DEM 28 billion (10,5% of its total expenditures) per year. Despite these potentially tremendous costs it is still a fairly obscure disease in terms of proper diagnosis and lacking effective treatment. Objective: To assess diagnosis and treatment options of IBS using strict Rome-II criteria. This figure correlates well with previous findings. The patients responded to experience an average of 7 episodes per year, each lasting about 4-5 days. Of the physicians questioned, only 73% recognize IBS when given the symptoms; 57% of these actually classify it accordingly while an alternative diagnosis is "irritable colon" (24%). Although it is sometimes merely regarded as a functional disorder - the common cold of the bowel -, IBS constitutes a severe illness and is said to be the second most common cause for work absenteism. Almost one third of the patients25. Bowel Diseases And Disorders: Inflammatory Diseases IncludUlcerative colitis and Crohn's disease are the commonest of a group of potentially debilitating chronic inflammatory bowel (intestinal) disorders, mostly of uncertain cause but with generally similar effects. Ulcerative colitis is an inflammation of the colon (the large or lower bowel). Crohn's disease (also known as regional enteritis) more often affects the lower section of the small bowel (ileum), but may involve any part of the digestive tract. Crohn's disease may also result in a deficiency of vitamins and minerals because it often affects the small bowel where these are absorbed. This deficiency is not usually a problem with ulcerative colitis because its effects are confined to the large bowel, which has a different function. The majority of people suffering from inflammatory bowel disease manage to cope unaided with the essential activities of daily living, even during relapses, and have no significant mobility problems. The necessity for surgical treatmentOverlap between CFS and other symptom-based syndromesThere are a number of conditions characterised by a similar and somewhat overlapping symptomatology to Chronic Fatigue Syndrome (CFS), such as: Given the overlap between conditions, patients with CFS may fit the criteria for two or more of the other syndromes. Often the dominant symptoms will determine which syndrome each patient is placed under. Clinical recognition of multiple syndromes is important to ensure the most effective clinical management and treatment plan. Additionally, there are broad psychological and social implications of the carrying the burden of being labelled with multiple syndromes; patients receiving a diagnosis of both CFS and FM have been shown to have significantly higher rates of unemployment and disability than patients with only one disorder, These syndromes each have clinical research case definition criteria, similar in purpose to those designated for CFS, which also can be modified for clinical diagnosis by a physician.
