This essay argues against this strict division, using examples and data mainly referring to gastrointestinal func-tional somatic syndromes (psycho-somatic disorders). The ef-fects of context and also of psychotropic agents on this interac-tion between psyche and soma is discussed. At this stage you may ask: `What has this got to do with bowels or their contents?". In clinical practice this is evidenced by the multitude of psy-chosomatic disorders and syndromes, which produce functional somatic symptoms. The gastrointestinal tract is frequently ef-fected, e.g. aerophagy, irritable bowel syndrome (IBS), non-ulcer dyspepsia, but other systems may also exhibit functional somatic symptoms e.g. fibromyalgia, tension headache, low back pain, chronicCONSTIPATIONDifficult, uncomfortable, or infrequent bowel move-ments that are hard and dry. In most people, constipa-tion is harmless, but it can indicate an underlying disor-der. Any of the follow-ing may be a sign of constipation: Sit at least 10 minutes, whether or not a bowel movement occurs. - Drinking hot water, tea or coffee may help stimulate the bowel. Hold the fluid inside until you are uncomfortable, then sit on the toilet for a bowel movement. ACTIVITY Regular exercise and good physical fitness help stimu-late the bowel and maintain healthy bowel patterns. - Constipation persists despite self-care, especially if the constipation represents a change in your normal bowel patterns. (Changes in bowel patterns may be an early sign of cancer).M3332-05This is the perception of sensory stimuli at a distance from its source (e.g. acute cholecystitis causing diaphragmatic irritation with the patient feeling pain over the right shoulder tip). Review other symptoms referable to the abdominal system: When were the patient's bowels last opened and when was flatus last passed? There may be a positive family history of inflammatory bowel disease or bowel carcinoma. Enquire about inflammatory bowel disease, carcinoma of the bowel and coeliac disease. Find out what the normal pattern of bowel movements are for the patient. A very short history of a few hours is likely to indicate an infective aetiology, whereas altered bowel habit for many years is more likely to indicate irritable bowel disorder in a young patient. If it is associated with blood, the most likely causes are inflammatory bowel disease or colorectal carcinoma. If mucus or slime occurs in isolation, irritable bowel syndrome may also be a cause.
